Managing medical conditions is crucial to preventing pain and discomfort for patients coping with the final phases of a terminal diagnosis. Our physician directs and advises the rest of the medical team so suffering doesn’t get in the way of our patient’s last opportunities to engage in what’s meaningful to them.

Hospice is so much more than just administering medication. Our nurses provide loving guidance to patients and their families as they navigate the end-of-life journey. Our nurses provide education on the patient’s terminal condition so families can provide the most support, help and comfort to their loved ones every step of the way.

When patients and families are focused on day-to-day challenges, our social worker adds a layer of support addressing big-picture details. Expertly trained in providing support to the caregivers, Capstone Social Workers connect families to useful resources, strategies for relieving the stressors of the caregiving experience and much more!

Our Chaplain provides support for our patients and their families in both spiritual and non-spiritual ways.

Near the end of life, it’s harder for patients to engage in what’s most meaningful. Our Director of Patient Services works with the team to help families stay engaged in what matters until the very end.
Most people have the perception that hospice is only the final stage of life, typically after the patient is confined to a bed, lasting one or two weeks while the body shuts down.
But for families who have the ability to engage hospice before the body is shutting down, there is more opportunity to engage in meaningful ways .

For adult children, the caregiving experience is a way to honor your parents in an incredibly meaningful way. One aspect of this experience is bathing your loved one. But for an elderly patient that could be experiencing pain and discomfort, our skilled nursing aids lovingly adopt this duty to provide the most comfortable experience.
